Những đóng góp mới của luận án

- Tỷ lệ xuất hiện: Các động mạch thắt lưng L1 – L4 hiện diện với tỷ lệ 97,8 – 100%, trong khi động mạch thắt lưng L5 chỉ gặp ở 17,8% trường hợp.

- Nguyên ủy: động mạch thắt lưng L1 – L4 chủ yếu xuất phát từ mặt sau động mạch chủ bụng (94,45 – 98,89%); ghi nhận các biến thể nguyên ủy từ mặt bên ĐMCB, động mạch chậu và động mạch cùng giữa (chủ yếu ở L4–L5). Ngoài ra, có các trường hợp chung nguyên ủy giữa hai ĐMTL hoặc thiếu hụt ĐMTL được bù trừ bởi các nhánh lân cận.

- Hướng đi: có xu hướng chếch lên ở động mạch thắt lưng L1 – L3, và song song với trục ngang ĐS hoặc chếch xuống ở L4 – L5.

- Đường kính: tăng dần từ động mạch thắt lưng L1 đến L4, trung bình 2,01 ± 0,53 mm (L1) đến 2,34 ± 0,53 mm (L4), và tương quan tuyến tính thuận với đường kính động mạch chủ bụng tại các mức L2 – L4 (p < 0,05).

- Chiều dài: các động mạch thắt lưng từ L1 đến L4 có chiều dài từ nguyên ủy đến bờ trong cơ thắt lưng lớn từ 29,24 – 31,15 mm; và từ nguyên ủy đến vị trí phân nhánh đầu tiên từ 44,18 – 46,82 mm.

- Tất cả các động mạch thắt lưng đều có ba nhánh chính; nguyên ủy nhánh tủy sống loại I chiếm ưu thế và tăng dần từ L1 đến L5 (30,34% lên 56,25%), trong khi loại II có tỷ lệ ổn định và loại IIIb là biến thể hiếm; tần suất nhánh phụ của nhánh lưng tăng rõ ở các mức thấp, đạt 88,9% tại L3 và 100% tại L4.

- Nguyên ủy động mạch thắt lưng L1 – L2 thường nằm ở phần giữa thân đốt sống, trong khi động mạch thắt lưng L4 – L5 nằm sát bờ trên thân đốt sống.

- Động mạch thắt lưng L1 – L2 nằm sau trụ cơ hoành trong 84,09% (trái) - 88,89% (phải); 100% động mạch thắt lưng đi sau và áp sát bờ trong cơ thắt lưng lớn, và sau chuỗi giao cảm thắt lưng.

- Mỗi động mạch thắt lưng thường có một tĩnh mạch thắt lưng tùy hành, không tương quan thuận với đường kính động mạch (p > 0,05).

- Nguyên ủy động mạch mạc treo tràng trên nằm gần nguyên ủy động mạch thắt lưng L1, nguyên ủy động mạch mạc treo tràng dưới nằm gần nguyên ủy động mạch thắt lưng L3.

- Tỷ lệ nhánh xuyên động mạch thắt lưng lần lượt là 95,55% tại L1, 96,67% tại L2, 96,66% tại L3 và 97,77% tại L4; tổng cộng ghi nhận 368 nhánh, trung bình 4,09 nhánh/mẫu (một bên).

- Nhánh xuyên vách – da chiếm 82,88%, trong khi nhánh cơ – da chỉ chiếm 17,12%. Trong đó nhánh vách – da gặp nhiều nhất ở L1 và L3.

- Đường kính nhánh xuyên (giá trị thực) dao động từ 1,15 ± 0,23 mm (L1) đến 1,24 ± 0,29 mm (L4); không có khác biệt giữa hai bên (p > 0,05).

- Chiều dài nhánh xuyên từ gốc mỏm ngang đến lá sau cân thắt lưng giảm dần từ L1 đến L4; chiều dài từ cơ dựng sống đến lá sau cân thắt lưng dài nhất ở L2 (38,13 ± 15,22 mm).

- 100% nhánh xuyên xuất phát từ nhánh lưng của động mạch thắt lưng và đi kèm hai tĩnh mạch tùy hành.

- Vị trí nhánh xuyên đi vào cân thắt lưng có phân bố tương đối ổn định: khoảng cách đến đường mỏm gai từ 68,30 mm (L5) đến 74,66 mm (L2); khoảng cách đến đường nối hai gai chậu sau trên (PSIS) lần lượt là 93,01 mm (L1), 78,64 mm (L2), 58,85 mm (L3), 34,88 mm (L4) và 26,86 mm (L5).

New conclusions of the thesis

- Presence rate: Lumbar arteries L1–L4 were present in 97.8–100% of cases, whereas the L5 lumbar artery was identified in only 17.8%.

- Origin: Lumbar arteries L1–L4 originated predominantly from the posterior surface of the abdominal aorta (94.45–98.89%). Variations of origin from the lateral aspect of the abdominal aorta, the iliac arteries, and the median sacral artery were observed, mainly at L4–L5. In addition, common trunks between two lumbar arteries and absent lumbar arteries compensated by neighboring branches were recorded.

- Course direction: The arterial course tended to be obliquely ascending for L1–L3, and nearly horizontal or obliquely descending for L4–L5.

- Diameter: The mean diameter increased progressively from L1 to L4, from 2.01 ± 0.53 mm (L1) to 2.34 ± 0.53 mm (L4), and showed a significant positive linear correlation with the abdominal aortic diameter at L2–L4 (p < 0.05).

- Length: For L1–L4, the length from the arterial origin to the medial border of the psoas major ranged from 29.24 to 31.15 mm, and the length from the origin to the first branching point ranged from 44.18 to 46.82 mm.

- Branching pattern: All lumbar arteries exhibited three main branches. Type I spinal branch origin was predominant and increased from L1 to L5 (30.34% to 56.25%), whereas Type II remained relatively stable and Type IIIb was rare. The frequency of accessory branches of the dorsal branch increased markedly at lower levels, reaching 88.9% at L3 and 100% at L4.

- Topographic origin level: The origins of L1–L2 lumbar arteries were commonly located around the mid-portion of the corresponding vertebral body, whereas L4–L5 origins were situated close to the superior border of the vertebral body.

- Relationships to surrounding structures: Lumbar arteries L1–L2 coursed posterior to the crus of the diaphragm in 84.09% (left) to 88.89% (right). All lumbar arteries passed posterior to and closely along the medial border of the psoas major, and posterior to the lumbar sympathetic trunk.

- Accompanying veins: Each lumbar artery typically had one accompanying lumbar vein; venous diameter did not show a positive correlation with arterial diameter (p > 0.05).

- Proximity to mesenteric arteries: The origin of the superior mesenteric artery was close to the origin of the L1 lumbar artery, whereas the origin of the inferior mesenteric artery was close to the origin of the L3 lumbar artery.

- Perforator prevalence: The prevalence of lumbar artery perforators was 95.55% at L1, 96.67% at L2, 96.66% at L3, and 97.77% at L4. A total of 368 perforators were identified, with a mean of 4.09 perforators per specimen (per side).

- Perforator type: Septocutaneous perforators accounted for 82.88%, whereas musculocutaneous perforators accounted for 17.12%; septocutaneous perforators were most frequently observed at L1 and L3.

- Perforator diameter: The true perforator diameter ranged from 1.15 ± 0.23 mm (L1) to 1.24 ± 0.29 mm (L4), with no significant side-to-side difference (p > 0.05).

- Perforator length: The length from the root of the transverse process to the posterior layer of the thoracolumbar fascia decreased from L1 to L4; the length from the erector spinae to the posterior layer of the thoracolumbar fascia was greatest at L2 (38.13 ± 15.22 mm).

- Origin and venous accompaniment: All perforators arose from the dorsal branch of the lumbar artery and were accompanied by two venae comitantes.

- Fascial entry point (localization): The perforator entry into the thoracolumbar fascia showed a relatively consistent distribution: the distance to the spinous process line ranged from 68.30 mm (L5) to 74.66 mm (L2); the distance to the line connecting the bilateral posterior superior iliac spines (PSIS) was 93.01 mm (L1), 78.64 mm (L2), 58.85 mm (L3), 34.88 mm (L4), and 26.86 mm (L5).
